All applicants must have criteria mandated by the state of Florida including:

• Professional Certificate of Completion with the CURRICULUM on the back (HHA,CNA, LPN, RN)
• Initial (Original Certificates) and C.E.U.’s (New Certificates) for HIV / OSHA and Domestic Violence
• I.D. and Social Security Card
• Proof of Residence / Employment Authorization
                      or
• Proof of Citizenship: Passport, Birth Certificate, or Naturalization
• Auto Insurance (if driving)
• Physical (including TB or Chest X-ray) within the past 6 months from a Medical Doctor detailing that the patient is “free of communicable diseases.”
• 3 Business References
• CPR Card
• Statewide background screening (if one has not been done within the last year the cost is $30 to be paid at the point of application)
• At least one year of experience required as an HHA, CNA, LPN, RN
